 The FREE clinic had equipment like this as late as '91 in Duluth, MN. And the hospital I worked in in the late '70's had this equipment in their storage area ready to use if needed. But am betting this pic was taken with equipment bought used from a second-hand store, prob by someone into some kinky things (I bought a gyno-table from a second-hand store back in '86 for my GF & I to use for our funn, gave it to her when I moved, and she was still using it 5 years later !)
